We know that we ask for a lot of information from you as we prepare for the festival. The below outlines some of the key elements of our various forms and hopefully helps you through some of the sticker parts. If you’re still having trouble, you can contact your Producer at any time.
A crucial form to help us get all the bits of info we need in order to complete your contract and get you ready for payment.
Some info that might be helpful to know:
It’s a Google Form – if you would prefer not to complete a Google Form, you can download alternative formats of the forms below. Just remember that we need ALL the information – when forms are returned incomplete, it slows down the contracting and sometimes, the payment process.
You can upload other information into the form as attachments. For example, your New Vendor Form. In order to do this, you’ll need to be logged into a Google account. If you’d prefer to complete the form as a guest, you can email any additional forms you might need to add through to your Producer.
If you have any issues uploading any attachments, you can email them to your Producer or admin@placemakers.org.au

You don’t have to duplicate all your existing work – if you have a great Marketing Pack already, you can send this to us and note in the Google Form where in your pack we should pull this information from.
Please note: We often ask for information in very specific ways, so please be sure the info in your Marketing Pack is formatted in this way, otherwise we’ll have to ask you to do the form again.

We hope that most parts of the Vendor Request Form (sometimes referred to as the New Vendor Form) are fairly simple to complete. Here are some parts we’ve had questions about:
Date of Birth: We require this from sole traders in order to process their superannuation. This is mandatory as part of the super process. We’re not sure why the ATO doesn’t ask for it on the Super Choice Form….but for now, we’ll need you to complete this section.
Payment Terms: Set a period of time you traditionally would like your invoices to be processed within. If you have deliverables attached to your payments, these will also need to be completed before we can observe your desired payment term.
Super Choice Form: This is a requirement for sole traders invoicing for their labour. If you’re unsure about whether your engagement with us is subject to super, you should contact your Producer.
The Super Choice Form is a separate form you’ll need to download, complete and send to us – either via the Google Form or email.
Superannuation is processed alongside each payment we make to you, so we will need you to have submitted your form prior to your first payment.
Do I have to send you a Super Form? Yes – in many cases, its a legal obligation. We also must put your contribution into a specific Super Fund – we can’t just pay it to your bank account. This is why we need your ATO Super Form.
Not sure if you need to send us a Super Form? Generally, if you are a Sole Trader, super will be payable, and listed as included in your fee. If you’re unsure, refer to you contract or to your Producer.
As part of our security processes, we require verification of your bank account details in one of the following formats:
a) Bank Statement screenshot, with business name, BSB and account number visible.
b) Company Letterhead with Bank Account Details
c)Bank Deposit slip

HERO IMAGE(S)
Remember, we ideally need horizontal and vertical versions of a hero image. This is to accomodate all the idfferent formats and platforms we'll be communicationg about your show on!
Images need to be high resolution with a minimum size of:

You need to be signed into the form a Google account in order for the form to save your progress.
A Google account email doesn’t always have a gmail.com domain – if you have a Google Drive, you have a Google account, even if its with another email provider.
